WHAT IS SELF DIRECTED FUNDING?

 
Self directed funding is essentially about three things for people living with a disability and their families;  independence, choice and social inclusion. It places the individual at the centre of decision making.
People with a disability and their families can become active participants in the design and delivery of their disability support. With the assistance of a host provider, they can plan, purchase and select supports and services that meet their individual needs.
Your Life Your Choices allows choice between: receiving services that are delivered by funded non-government providers in the traditional way; or to self-direct their supports designed by them to suit their needs specifically.

MODELS TO CHOOSE FROM

 

MODEL 1 – 6.5% OF THE PACKAGE IS HELD BY ABILITY CARE LTD.

Services provided by Ability Care Ltd
–  Initial Assessment – such as Initial home visit and interview of the client/s, their carers and/or family member/s.
–  Collaboratively design a self directed action plan to incorporate needs, goals and desired outcomes.- Ability Care Ltd acts as administrator to transfer the funds to client’s account on a monthly/quarterly basis as agreed within the action plan.
–  Ability Care Ltd will be responsible for financial and Departmental acquittals.
Client and Family Responsibilities
 

MODEL 2 – 10% OF PACKAGE IS HELD BY ABILITY CARE LTD.

Services provided by Ability Care Ltd
– Ability Care Ltd acts as administrator and holds funds to make all payments (support worker and other brokerage services) at the direction of the client/family in accordance with the action plan.
– Ability Care Ltd will be responsible for financial and Departmental acquittals.
 
Client and Family Responsibilities
– Client/Family will be responsible to source and action all supports and services.
– Client/Family will be responsible to ensure all invoices for costs incurred are forwarded to Ability Care Ltd to action.
– Client/Family will be responsible to manage their expenditure in accordance to the action plan and provide information to Ability Care Ltd for financial acquittals.

MODEL 3 – 5% OF PACKAGE IS HELD BY ABILITY CARE LTD.

In addition to Model 2, Ability Care Ltd will assist with:
– Staff Recruitment, management, ongoing training and professional development on your behalf.
– Brokerage Services – finding the right services, support and equipment for you.

FEE FOR SERVICE – $85 PER HOUR

Client/family may seek additional services through fee for service. Services offered include but not limit to:
– HR and IR Assistances
– Linking and Referral
– Individual Support Plans – Case Planning, Co-ordination, Review and Facilitation.
– IT Support

WHO IS ELIGIBLE?

Queenslanders with a disability aged 0-65 who receives ongoing individualised funding from Disability Services.

AVAILABLE IN

Queensland: Metro, regional, rural and remote areas.

FEE STRUCTURE

An initial $1000 fee for assessment, (inclusive of home, visit, interviews) and design and development of case plan in line with assessed need.
Model 1 – 6.5% of total package
Model 2 – 10% of total package
Model 3 – 5% of total package
Fee for Service – $85 per hour
